People living in care homes should expect the same level of support as if they were living in their own home. This can only be achieved through collaborative working between health, social care, Voluntary, Community, and Social Enterprise (VCSE) sector and care home partners.


The Enhanced Health in Care Homes (EHCH) model moves away from traditional reactive models of care delivery towards proactive care that is centred on the needs of individual residents, their families and care home staff. Such care can only be achieved through a whole-system, collaborative approach.


I am supporting the 13 elderly care homes. Visiting the homes at least monthly and completing Individual Personalized Advanced Care Plans for all the 510 residents - speaking with both the residents and their power of attorney. Advance care planning can make the difference between a future where a person makes their own decisions and a future where others do. Proxy medication ordering is being rolled out for the care homes, and I am supporting the Homes along with the Medicine Team. This process will have time and cost saving for both the GP surgery and care home. It will mean fewer errors with ordering, issuing, collecting, and dispensing medications. Improve clinical safety, with an easy access to audit trail.
